HELP: Online Pass

I just got the game yesterday and when I typed in my codes, they says it's been used already/incorrect codes.
Anyone knows why?

A tip for anyone having issues activating their Online Pass in FIFA 11 on X360. Once entered, relaunch the game from dashboard.
